At Chet's we are committed to delivering the highest quality care to our patients and customers. Because of the ever increasing demands on your time, we have several options available for you to request your prescription refills.

Telephone in your prescription refill request to: 715-268-8121 or 888-539-9210
After our regular business hours, you may simply leave a voice mail message with your prescription numbers and name, and they will be ready for pickup the following morning or fill the form below.
The prescription number is the 7-digit number in the upper left-hand corner of the label. If you do not have your bottle close at hand when requesting the refill, you may also give us the drug name and we will look up the prescription information on our computer files.
Please bring your driver's license and insurance cards with you when picking up prescriptions.
